What has happened

Two near misses this month involved the permit system: the lube oil pump missing from the BFP-2B isolation list, caught at walk down, and a contractor scaffolder entering the GT1 fuel gas skid area without checking the permit board while the skid was depressurised for instrument work. The scaffolder event has no corrective actions recorded. Jordan Meyer wants the September permit audit done early, with every permit issued this month reviewed against the policy and the multiple energy source procedure, breaches recorded as incidents, and corrective actions with owners before the October outage brings 90 contractors to site.
